Founded in 2021, Fountain Home Health L.L.C. prides itself on delivering high-quality care to our clients. We are dedicated to meeting the evolving needs of those we serve and continuously improving and innovating our services.
Our team of healthcare professionals includes registered nurses (RNs) and certified nursing assistants (PCAs), who are trained to provide both skilled and non-skilled services. We also offer home health aide services that are tailored to meet the specific needs of each patient. Our staff is dedicated to helping patients maintain their independence in all activities of daily living (ADLs) while providing as much assistance as necessary, including companionship, light housekeeping, meal preparation, and diabetes care. Our team approaches each patient with kindness and compassion.
Our care team continuously stays on top of the ever-changing healthcare needs through continuing education training that is accredited by the American Nurse Credentialing Center (ANCC) through Nurse.com.
Fountain Home Health L.L.C. collaborates with physicians, case managers, hospitals, insurance carriers, or other agencies for holistic care of our clients.
